How to Submit a Request

Follow this process if:

  • you are able to visit us at 456 W. Main St. (Mesa) or 2432 W. Peoria Ave (Phoenix), 4695 N. Oracle Rd (Tucson) and
  • you are a US Citizens or Permanent Resident

The entire Livescan process in our office takes about 15 minutes. When finished, you will leave with an on-line access code that will allow you to view your FBI Report.  The access code will allow you to print and download a copy, as well.

In addition to the electronic copy (pdf), you may also upgrade the service to receive an authenticated, OFFICIAL FBI Criminal Report. The official FBI Report is printed on security/tamper, and copy proof paper with the FBI logo, FBI seal watermarked on the report and authorized FBI official’s signature. The authenticated paper report will be sent out to you on the same day (Monday-Friday) by USPS 1st Class Mail with a 4-6 day arrival. You may also upgrade shipping to USPS Priority Mail (2-3 Days) or FedEx Overnight (by 3 pm next business day). This is the copy you will need to help expedite an FBI Report that needs to be Apostilled.

WHY CHECK MY FBI CRIMINAL REPORT?

Personal Review– Reviewing your official FBI Identification Record, and correcting inaccurate information, could put you on a new career path in today’s security-conscious work environment.

Foreign Travel or Work– Some foreign countries/embassies ask that you provide a copy of your criminal history record when requesting travel authorization.

Work or Student Visas– When applying for a Work Visa or Student Visa, some foreign countries require a copy of your criminal history record as part of the background investigation.

Adoption Requirements– Most states and some foreign countries require a complete criminal history check when applying to adopt a child.

Attorney Requests– To ensure proper legal representation, federal regulations allow you to request a copy of your personal criminal history report and have it sent directly to your attorney.

Challenge Errors and Inaccurate Information– Just like your personal credit report, you have the right to challenge inaccurate information on your FBI Criminal Report and have it corrected.

Court Related Matters– With arrest, conviction and sentencing information from all 50 U.S. States, the District of Columbia and the U.S. Territories of Puerto Rico, Guam and the Virgin Islands, the Criminal Justice Information System (CJIS) at the FBI is very busy. Federal courts report conviction and sentencing information directly to the FBI. State and local courts report their information to a state identification bureau, which is responsible for sending the information to the FBI.

Please be aware of the rules and regulations governing how FBI criminal history information may be requested and utilized. The fee for fingerprinting and report is $129.
